Patient Registration
Your registration will be with the Nuffield Practice. Please let the receptionist know if you have a particular preference. Otherwise, one of the doctors will be allocated as your preferred doctor. We feel that it is very important for continuity of care that you see your usual doctor wherever possible. You may ask to see a different doctor and we will do our best to respond to your request (although this may not always be possible.)
When you register you will be asked to fill out a registration form and a medical questionnaire, available from Reception. The questionnaire is essential because it can take a considerable time for us to receive your medical records.
If you are already on repeat prescriptions it is very helpful if you bring a copy of your repeat prescription form when you register.
